Programs at Camp Powhatan

Medical Forms

All Scouts must complete the BSA Medical Form, parts A, B, & C (see below). The physical must have been completed within 12 months prior to the end of camp (June 30) to be valid.

You must also include a copy of both the front and back of your insurance card.

We will be collecting forms on Monday June 3 at the regular meeting. If you bring forms when we are packing up on Friday, June 14, please bring 6 copies of each document as we will not have sufficient time to copy them at that late a date and have to provide them to various programs.

Make sure we have the following:

  1. Fully completed and updated medical forms
  2. Copies of front and back of insurance cards
  3. Consent form (attached) signed by both parent and child

In addition, please make sure that we have phone numbers we can use to start the phone tree when the troop leaves summer camp on their return trip on Saturday, June 29th. I will have a sign up sheet at ??? or you can email me if you will not be attending the meeting (if you provided this information to me at the Parents' Meeting on 5/6/13 you do not need to re-send it).

You can find out more about the BSA medical requirements on this page.
