All Scouts must complete the BSA Medical Form, parts A, B, & C (see below). The physical must have been completed within 12 months prior to the end of camp (July 19) to be valid.

You must also include a copy of both the front and back of your insurance card.

We will be collecting forms on Monday June 2 at the regular meeting. If you bring forms when we are packing up on Friday, July 12, please bring FOUR copies of each document as we will not have sufficient time to copy them at that late a date and have to provide them to various programs.

Make sure we have the following:

  1. Fully completed and updated medical forms
  2. Copies of front and back of insurance cards
  3. Consent form (attached) signed by both parent and child

You can find out more about the BSA medical requirements on this page.

What to bring:

Camp Durant provides a list of what to bring to camp.

We also have our own list: Troop 486 Summer Camp Checklist
